Project Officer - Life Without Barriers (Adelaide / All Adelaide)

A fantastic opportunity exists for a Project Officer to join the Partners In Recovery (PIR) team in Christies Beach on a contract basis until 30/6/2016 for 22.8 hours a week with possibility of extension due to funding.
Partners in Recovery is a Commonwealth initiative that aims to better support people with severe and persistent mental illness with complex needs and their carers and families, by co-ordinating multiple sectors, services and supports they may come into contact with (and who could benefit from) to work in a more collaborative, coordinated and integrated way.
The Project Officer will (in addition to the responsibilities listed in the Position Description);
Focus on the continued development of our Physical Health Needs Program which looks at coordinating services for clients with co-morbid mental health and physical health needs. Develop Policy and Procedures as required by the program. Partnership development and stakeholder relations across clinical and physical health needs providers. Provide leadership in the development of a Physical Health Needs service model, assessment tool and evaluation tool. Identify and improve referral pathways to facilitate access to services and supports for consumers with complex mental health and physical health needs. Other PIR project activity. Essential Selection Criteria :
Relevant qualifications in a health or project related discipline. Demonstrated experience in change management. Ability to prepare and interpret evaluation reports. Desirable Selection Criteria :
Demonstrated experience in working with a mental health service in a community setting. Experience working in the primary health care sector. Knowledge of national directions in terms of chronic disease management and mental health service delivery. Knowledge of primary health care practice. All successful candidates will be required to undergo probity checks including Working with Children Check (where relevant to the position).
