The Role
We are seeking an experienced Registered Nurse-Team Leader in Community Care, who is looking for a an exciting Full time opportunity(maternity leave cover until June 2016) with a large in home Healthcare provider.
Function
To provide line management and clinical direction to Registered Nurse Case Managers.
Ensuring best practice in clinical care and to develop positive working relationships with RN Case Managers to facilitate person-centred and reablement focussed service delivery.

The Criteria
In order to be successful for this position, you must meet the following criteria:
AHPRA Registration Registered Nurse Experience in aged/community sector Experience in employee management and supervision Have excellent clinical and communication skills A genuine interest in providing clinical care in the home Experience in the management of Home Care Packages (desirable) Experience in the provision of complex Case Management and Clinical Care services and assessment (desirable) Excellent interpersonal skills Excellent computer skills The ability and desire to work autonomously Drivers licence and reliable vehicle
